Hypoactive Sexual Desire Disorder (4 questions, 28 members) Since 1998, when “the little blue pill” called Viagra created a revolution in treating male sexual dysfunction, expectations have been high that “the little pink pill” – an equally powerful medication to help women — will follow soon. The task, however, has proven to be more difficult than originally expected – as of now, not a single compound has been officially approved to treat sexual dysfunction in women. Several compounds, vying for the title of “the little pink pill”, are in different stages of development and some of them may hit the market as soon as this year. Here’s what every man needs to know. How is sexual dysfunction in women different from sexual dysfunction in men? The most common form of sexual dysfunction in males is Erectile Dysfunction (ED), i.e., inability to develop and sustain erection. In other words, affected men want to have sex, but cannot perform.

Erectile dysfunction

Flibanserin for Premenopausal Hypoactive Sexual Desire Disorder: Pooled Analysis of Clinical Trials. The most common type of female sexual dysfunction is Hypoactive Sexual Desire Disorder (HSDD). Women with HSDD can perform in bed and even reach orgasms; however their libido, i.e., interest in sex is low or non-existent. HSDD may be life-long or situational, caused by, for instance, birth and nursing of a child. In 2013, New York Times suggested that up to 30% of otherwise healthy women between the ages of 20 and 60 are affected. Sprout Pharmaceutical and Palatin Technologies, two companies actively developing medications to treat HSDD, report 43% and 12% respectively.
5.2 Prolonged Erection and Priapism
Using Addyi In patients with liver impairment may lead to severe low blood pressure and fainting. Addyi should not be used in these settings. Ask your doctor or pharmacist if you are not sure if you have liver disease. Dizziness, tiredness, and nausea in about 10% to 11% of patients (vs. 2% to 4% receiving a placebo).
Other Interactions
Addyi can increase the risk of sleepiness if taken during waking hours. Fatigue, trouble sleeping and dry mouth have also been reported. The risk of severe low blood pressure and fainting may occur when Addyi is used alone or when combined with alcohol or certain other medicines or herbal supplements. Do not drink alcohol close to the time you take your Addyi dose because this increases your risk of severe low blood pressure and fainting (loss of consciousness). If you take Addyi and feel dizzy or lightheaded, lie down right away if you feel like you could faint. Even at the lower end of the range, it is likely that well over ten million women in the USA alone experience HSDD.

6.1 Clinical Trials Experience
There is also a Boxed Warning for using Addyi with certain medicines called moderate or strong CYP3A4 inhibitors. Taking these drugs together can increase the blood levels of Addyi in your body and could increase your risk of low blood pressure, sleepiness or fainting (loss of consciousness). Ask your doctor or pharmacist if you are not sure if you take drugs known as CYP3A4 inhibitors. Medicines you may need to avoid include certain: Heart, chest pain or high blood pressure medicines Herbal supplements, for example: St. John’s Wort, ginkgo, or resveratrol Over-the-counter (OTC) medicines such as cimetidine (Tagamet HB) Do not drink grapefruit juice if you take Addyi because this may also increase your risk of severe low blood pressure and fainting (loss of consciousness).
Less common
Tell your healthcare provider about all of the medicines you take, including prescription, over-the-counter (OTC) medicines, vitamin or herbal supplements. Do not take or start taking any medicines while taking Addyi until you have talked with your doctor. These combinations may not be safe and increase your risk of severe low blood pressure and fainting. Learn more about Addyi drug interactions (in more detail) here. Do not take Addyi if you have liver problems. Hypoactive women describe their symptoms as “declining interest in sex”, “loss of spark”, “always not in the mood”, “lack oral sildenafil of passion”, or “being unexcited”, regardless of whom they are with and how long they’ve been together.

It goes without saying that proper diagnosis must be left to a trained physician, preferably somebody who has experience in female sexual health. Unfortunately, many women consider declining desire “a fact of life” and try to hide it in order “not to disappoint my partner”. Low desire is a medical condition and should not be ignored. In loving relationships, it is the role of the male partner to encourage his wife of girlfriend to seek medical help. Physiology and psychology of female desire is an exceptionally complex interplay of hormones and neurochemicals. What is clear, however, is that the cause of depleting libido is primarily in the woman’s brain, not her genitalia. In fact, HSDD is recognized as a mental disorder. This is one of the reasons why experimental treatments of HSDD with testosterone or Viagra-like substances showed limited effectiveness. As mentioned before, as of the time of this writing, there are no FDA-approved medications to treat HSDD. There are, of course, multiple “supplements” purporting to do wonders; however, their effectiveness and, more importantly, safety are unproven. At the same time, several promising medications are in FDA-supervised clinical trials.
6.2 Postmarketing Experience
A Boxed Warning regarding combining alcohol and Addyi use is outlined in the prescribing information and Medication Guide for the drug. A Boxed Warning is the FDA's most stringent safety warning. There are also Boxed Warnings regarding use in liver impairment (liver disease) and for certain drug interactions, as explained below. Combining Addyi with alcohol may increase the risk of low blood pressure and fainting (loss of consciousness), which is problematic especially if you are driving, doing hazardous activities, or need clear thinking or reaction times. Women should discontinue drinking alcohol at least two hours before taking Addyi at bedtime (if they've had 1 or 2 standard alcoholic drinks that night), OR Skip the Addyi dose that evening if they have had 3 or more standard alcoholic drinks that night.
Who should not take sildenafil?
If you skip a dose of Addyi, take your next dose at bedtime the next day. After taking Addyi at bedtime, do not use alcohol until the following day. Do not consume alcohol at least until the morning after taking Addyi at bedtime. One standard alcoholic drink contains 14 grams of pure alcohol and is equivalent to one 12-ounce regular beer, 5-ounces wine, or 1.5 ounces of distilled spirits or shot. These compounds (distant relatives of anti-anxiety medications) work on readjusting the interplay of serotonin and dopamine, two neurochemicals believed to be responsible for the desire. How promising are the results of clinical studies? Anecdotally, some participants in studies conducted at Boston Clinical Trials and other research centers reported dramatic improvement in their sexual appetites. Yet other participants experienced limited or no change. It is important to keep in mind that the studies were double-blinded, i.e., neither the patient, nor the staff members knew whether the person received an investigational drug or a placebo.

When should we expect to see the little pink pill on the market? In 2014, FDA identified HSDD as one of 20 disease areas of high priority. Given the FDA focus and the promise of a multibillion market, there is little doubt that the treatment for HSDD will become a reality soon.
